2008 UFCC Agency Application

The application form for use by agencies applying for inclusion in the 2008 UFCC can be found by clicking on the link below. This form is mandatory for all applications for the UFCC. It is suggested that any additional information that may be pertinent to the questions asked be attached as supplementary documentation, if necessary. All questions must be answered and all required up-to-date supporting documentation must be included to be considered for eligibility in the UFCC.

By December 1, 2007, the Planning Committee will publish, in a newspaper of general circulation in Alachua County, a notice informing organizations of the application procedures for participation in the UFCC to be held in the subsequent fall term.

Applications have been mailed to all agencies participating in the 2007 UFCC and other organizations that expressed an interest in the UFCC. Upon request of an organization wishing to participate, the committee will provide a copy of the governing procedures and an application form. All application materials and proceedings are subject to the Florida Public Records and Sunshine Laws.

The Planning Committee will review all applications received by the February 1, 2008, deadline date by May 2008. Organizations may expect to be notified of the decision of the committee on their applications no later than June 2008. Organizations will have ten days from the receipt of notification to file an appeal of the decision of the committee.
